Care Coordination

LAMC's Care Coordinators can be found everywhere - in the community. They work at the hospital, go to schools, help out at senior centers, and participate with other Northern New Mexico health professionals in providing screenings at such unlikely locations as casinos.

Care Coordination personnel – nurses and social workers – perform a number of important functions for our patients. Inside the hospital walls, they oversee the patient's discharge planning, seek appropriate resources to ensure that the patient gets what he or she needs upon leaving LAMC, and then follows up to make sure things are going well at home. In our communities, the Care Coordinators help train lay health advisors in clinics throughout Northern New Mexico and invest a great deal of their time in working with those who need disease management, in particular diabetes.

These dedicated nurses are charged with getting the word out to residents of Northern New Mexico -"LAMC wants you to stay healthy!"
